Do you need forms for your website? Maybe you need to collect additional information from your customer to complete their purchase. Dreams by Taylor offers form creations just for you! You may choose up to 10 questions for your form. Each form will be integrated with your website to match your sites look and feel. The form will be created with a submit button that will automatically send you an email with the forms contents. You may use these forms for: Newsletter Sign Up Box Stationary "Extras" Return Authorization Forms Detailed Contact Us Pages Customization Information Suggestion Box Wholesale Application (extra charge) Customized Form Please note: The cost per form is $15.00 for up to 10 form questions Drop down or list/menu boxes including state selection and/or country selection for use in a wholesale application will be extra. * Note that if you manually manipulate your form yourself and it doesn't work properly, we are not able to re-code it free of charge. You will need to order a new form service.
